The player is able to have children after they upgraded their house to level 3 and are in a marriage. If the player is married to the same sex, you will be able to adopt children. In current version you can only have 2 kids of different sex, and they won't past toddler stage.
Biological Children
If you maintain a good relationship with your spouse (hearts maxed out), already have the nursery, when you go to bed earlier than your spouse, he/she will ask whether you want a child. If the player agrees, after a month, a baby is born and put in the crib at nursery.
