Health is measured by a bar next to the player's energy bar in the bottom right corner of the user interface (UI). The health bar is only visible while inside of The Mines or when the player has been hurt. Outside of the Mines the only way to lose Health is by getting hit by the train on the Railroad or by the slimes in the Secret Woods. Health can be replenished by consuming food, sleeping or using The Spa. The maximum amount of health increases by five for each level gained in Combat skill.

Once the player's health reaches zero, they will pass out after which they will be woken up by Linus, Robin, or Harvey in the entry room of the mine, their bedroom, or in Harvey's Clinic, respectively. If awoken in the hospital, Harvey charges the player 1000g for surgery. If the player passed out due to health loss, they can lose some money, a random assortment of items, and can possibly lose knowledge of floors in the mine, requiring them to re-discover them again.
